
New Patient Information
During your first appointment, we will discuss your diagnosis and treatment options. In some cases, surgery may be done on the same day. However, if you have a complicated medical history or treatment plan, we may need to schedule a second appointment for treatment on a different day.
To help us prepare for your consultation, please provide the following information:
Please bring your surgical referral slip and any x-rays, if necessary.
Please provide a list of the medications you are currently taking.
Please bring completed insurance forms for faster processing.
IMPORTANT: Patients under 18 years old must have a parent or guardian present at the consultation.
A preoperative consultation and physical exam are required for patients receiving IV anesthesia for surgery. Please refrain from eating or drinking for eight hours before surgery and arrange for a responsible adult to drive you home.
Please inform the office if you have any medical conditions that may affect surgery (such as diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, etc.) or if you are taking any medications (such as heart medications, aspirin, anticoagulants, etc.).
